Taking Control of Your Health

    
The Taking Control of Your Health Program (TCOYH) program is a patient self-management program offered to MBGH employer members to support their efforts in reducing the unnecessary costs associated with managing chronic conditions. This program has the potential to reduce medical costs, reduce absenteeism, increase medication adherence rates and the overall health of participants.

This program is
built on a “mutual accountability” model that includes employer incentives of waived or reduced co-pays when participating employees agree to meet face-to-face with a qualified pharmacist coach at least four times a year. Meetings are focused on teaching participants the skills necessary to  monitor their condition and improve their health. Currently, the TCOYH program offers support for those with diabetes, cardiovascular disease and hypertension.

Offered to MBGH employer members, the TCOYH program is run in collaboration with the Illinois Pharmacist Association (IPhA) and MBGH’s purchasing affiliate, the Midwest Health Purchasers Foundation (MHPF).
