There is growing use of accumulator adjuster programs by PBMs today. These programs work by prohibiting manufacturer copay card support from being applied to a patient’s deductible and out-of-pocket max. While the plan draws down the full amount of the copay card, when the assistance runs out (usually in 3-4 months), patients are faced with a substantial deductible balance – especially for those in high-deductible health plans. This can result in non-adherence to drug treatment and greater demand for health care services.

Proponents of copay cards view them as promoting affordable access to specialty medications and view accumulator adjuster programs as a means to creating cost barriers to specialty medication use.

On the other hand, employers who use accumulator adjuster programs view copay cards as a way to encourage unnecessary use of higher-priced, branded drugs and undermine their intended benefit plan design.
