While recent transparency legislation represents progress, substantial gaps remain in an employer’s ability to fully evaluate PBM compensation structures, pricing practices, formulary decisions, and alignment with plan fiduciary obligations. As drug costs continue to rise, employers are under increasing pressure to balance affordability, access, quality, and workforce health outcomes. Organizations that successfully improve pharmacy benefits performance will be those that move beyond passive oversight and adopt a more active, data-driven, and strategic approach to PBM management.
